ah, good one, "Beyond the Realms of Death", forgot about it.

You want to hear something funny?
Mercyful Fate's song "Melissa" was one of my fave songs in the mid 1980s, and it was 3 or 4 years later when I finally heard Judas Priest's "Beyond the Realms of Death" and THEN heard the similarities!

Hmm, what constitutes a 'ballad' exactly?
I suppose, like most things, it comes down to how you perceive it personally.
Some of my favourites:

SAXON : Frozen Rainbow (1979)
(Not the first minute obviously, but thereafter it qualifies as a ballad)
(The lead break which begins at 3:07 is one of my all time favourites. So much feeling!)

MANOWAR : Mountains (1984)
(Epic & Atmospheric)

WARROR SOUL : The Losers (1990)
(Great lyrics & mood. Raise your glass & make a toast.)

CIRITH UNGOL : Maybe That's Why (1981)
(Spellbinding)

AEROSMITH : Kings & Queens (1977)
(I'm NOT trying to claim Aerosmith are 'Heavy Metal', but i think this song does fit the bill for this thread)

THE DEVIL'S BLOOD : Voodoo Dust (2008)
(Transcendental!)

Aerosmith WERE called Heavy Metal in the 1970s, so I have no problem with that :)
(they stopped with their change to being a pop band with the "Permanent Vacation" album in 1987).

Tons, i have to admit that most of the ones mentioned already are amongst my favorites (Saxon, Manowar,Blaspheme, Blacksmith, a million hearts by the mighty Faithful Breath, Emerald (Nld)...), nut here are some that come in mind and have not been mentioned yet (in my way of perceiving the term):

Tons of the classic ones such as: Accept - Seawinds, Judas Priest - Run Of The Mill Iron Maiden - Remember Tomorrow/Strange World, Rainbow - Rainbow Eyes, half the discography of Scorpions and so on hehe...
And some more:
Nation (Ger): Alone
Attack (Ger): Destinies Of War
Axton Pryte (Fra): Eternal Recital
Dab (Fra): Devant la loi
Emerald (US): Look to the Stars
Flotsam and Jetsam (US): Escape from Within
Metallica (US): Fade to Black
Northwind (Gr): Medea
Manilla Road (US): Empire
Cirith Ungol (US): Fallen Idols
Omen (US): Hells Gates
Penzzer (Fra): Vivre Sana Elles
Valhalla (US): Ship Of Dreams
Omega (UK): The Child
Crimson Glory (US): Lost Reflection
Agent Steel (US): The Traveler
and the list goes on...

But, maybe my all time favorite one is this masterpiece

There are so many good metal ballads. Some of them could be considered borderline but they at least start off as ballads. Here are a few of my faves (in alphabetical order):

Armageddon (Swe) - Grain of Sand (Embrace the Mystery)
Cerebus (US) - Longing for Home (Too Late to Pray)
Death Angel (US) - A Room with a View (Act III)
Fifth Angel (US) - Wings of Destiny (Fifth Angel), Broken Dreams (Time Will Tell)
Iron Maiden (UK) - Prodigal Son (Killers), Wasting Love (Fear of the Dark)
King Diamond (Dnk) - So Sad (The Puppet Master)
Lääz Rockit (US) - The Omen (Annihilation Principle)
M.A.R.S. (US) - You and I (Project: Driver)
Medieval Steel (US) - Echoes (The Dungeon Tapes)
Omega Point (US) - Was it Me? (Infinite Rhyme)
Overkill (US) - The Years of Decay (The Years of Decay)
Queensrÿche (US) - The Lady Wore Black (Queensrÿche EP)
Sacred Rite (US) - R.I.P. (Sacred Rite)
Scarlet Rayne (US) - Sands of Time (Theater Humanitarian)
Stranger (Ger) - I Hold You (The Bell)
Thynn Ice (US) - Life and Death (Thynn Ice)
Villain (US) - Just Close Your Eyes (Only Time Will Tell), Heal My Heart (Chequered Past)
Whiplash (US) - Last Nail in the Coffin (Ticket to Mayhem)

I'm probably forgetting so much more but these are what I could think of now. I'll add more later.

Jeezus Strappado! What on earth was that?!!
I'm hoping & praying that you added that song as a joke.
It's the only reasonable explanation.

I love the hell out of ballads, that contrast between soft and heavy always gets me. There's definitely a limitless amount of 70s hard rock bands that did some amazing stuff, but I only mentioned two or three. I'm probably forgetting some of my favorites but these were the ones that came to mind.

Hell - Depths of Despair
Budgie - Turn To Stone
Witchfynde - Moon Magic + Doing The Right Thing
X Japan - Voiceless Screaming
Mercyful Fate - Melissa
Seikima II - Neverending Darkness
Scorpions - In Trance
Dragonfly - Space Bound
Critic Ungol - I’m Alive
Manilla Road - Court of Avalon
Judas Priest - Before The Dawn
Bow Wow - Still
Badge - Something I’ve Lost
Chasar - Kings
Praying Mantis - Lovers to The Grave
Saxon - Rainbow Theme
Grim Reaper - The Show Must Go On
Legs Diamond - Woman
